Steps to get an official 2000 CoDSM membership card
There are a few things that we'll need to complete your order for a
membership card. We must have all of the following items completed
before we can begin processing your card. Sorry but
membership cards are available to CoDSM members in good standing only.

A picture of you. This will be a photo that has a good 'head shot'.
We can use either a photo that you've scanned and can send as an email
attachment or you can send a regular photo which we'll scan and send back
to you. If you elect to scan and send your photo, please use a pixel resolution
of 300dpi. Low resolution photos will yield poor image quality on

Place your order (click here for the order page).
The cost for the membership cards is $5. This is to cover the cost
of producing the cards only. Don't forget to include
your return address with your order.
NOTE: To keep costs down, we must make up the cards in batches of
10 (that's how many fit on one page of high quality photo paper).
So we have to 'queue up' 10 orders. Please feel free to email us if you
want to know how many orders are pending. |
